1. State Driver's License
2. US Passport
3. Military ID
4. State ID
5. DMV Driver's Permit
6. Permanent Resident Card

• When obtaining fingerprints for the Department of Health and Human Services (DHHS), you will be required to provide a Fingerprint Authorization Form from your employer/company. This form will include a 6 digit number specifically assigned to you that is required for processing.
